- Graphical abstract: Graphical abstract The current AHA/ACC HCM guideline recommendation for a primary prevention ICD is based on retrospective series that have important limitations. These include selection bias, the fact that 'SCD events' are appropriate ICD interventions for monomorphic VT in the vast majority of cases, and a high prevalence of major confounders in patients with apical aneurysm and SCD events.
