- Abstract : Background and purpose. : The purpose of this case series was to document running, high‐level mobility, and strength outcomes following an intense exercise program including underwater treadmill training (UWTT) for people with traumatic brain injury (TBI). Methods. : Four individuals with TBI participated in a 3‐phase, 15‐week program. Phase 1 (6 weeks) consisted of strength, flexibility, balance, and agility exercises, Phase 2 (6 weeks) consisted of UWTT, and Phase 3 (3 weeks) concentrated on running over ground. Outcome measures included the high‐level mobility assessment tool (HiMAT), lower extremity muscle strength, running distance, maximum running speed, and quality of running. Results. : Average over ground running speed increased from 2.7 m/sec (meters per second) to 3.8 m/sec. Average over ground running distance increased from 98m to 250m. Average HiMAT scores increased from 19.3 to 31.8. Changes in muscle strength and quality of running were inconsistent, although some trends emerged. Conclusion. : Clinicians should consider the inclusion of UWTT and high‐level mobility training for certain individuals with TBI to help them learn how to run again while simultaneously improving high‐level mobility skills.
