- Abstract : Background: Sarcopenia is a syndrome characterized by progressive and generalized skeletal muscle loss and hypofunction. It is associated with poor clinical outcomes in sepsis, acute respiratory failure, tumor, surgery, and liver transplantation. It is also discussed in some literature as a predictor of splenic abscess severity and mortality. As a common disease of intra-abdominal infection, the acute pyogenic liver abscess has been lacking relevant predictors, so this study was conducted to evaluate whether sarcopenia can be used as a predictor of pyogenic liver abscess. Methods: This is a retrospective study of patients with a pyogenic liver abscess who were hospitalized and underwent CT examination between January 2014 and June 2021. We divided 59 patients into the following groups: 20 with sarcopenia and 39 without sarcopenia. Evaluation for sarcopenia was performed via calculating the psoas cross-sectional area at the level of the third lumbar vertebra and normalising for height before checking it against pre-defined values. Sarcopenia was noted if the values fell below gender-specific cut-offs of 385 mm2/m2 and 545 mm2/m2 for female and male patients. Laboratory tests, severity index and prognostic indicators were used to evaluate the differences between the 2 groups. Results: First of all, between the 2 groups, we found no significant differences in demographics such as sex, age and BMI (P-Value range from 0.0681–0.8981). Secondly, we found no differences not only in laboratory tests such as WBC, hemoglobin, CRP, PCT and total bilirubin (P-Value range from 0.4029–0.8918) but also in severity index such as QSOFA and SIRS (P-Value 0.9661 and 0.6439). Finally, we found no differences in prognostic indicators such as length of hospital stay (P-Value 0.9744). Conclusions: Predictors of liver abscess are currently lacking, and although sarcopenia is associated with poor clinical outcomes mentioned above, this association was not found in this study. The consideration may be related to the limitation of the small number of samples in this study and patients who underwent early percutaneous transhepatic abscess drainage. More research is needed to further evaluate predictors of pyogenic liver abscess. … (more)
