Abstract

Abstract : Background: Liver cancer is one of the most common cancers and a leading cause of cancer death. It has an aggressive disease nature and poor survival rate, resulting in a substantial cancer burden globally. We aimed to investigate the most updated global incidence of liver cancer by histological subtypes using international and national cancer registries. Methods: The data on the incidence of liver cancer by histological subtypes in 2020 were estimated based on WHO Cancer Incidence in Five Continents and Global Cancer Observatory. Case number and incidence of hepatocellular carcinoma (HCC) and intrahepatic cholangiocarcinoma (iCCA) were calculated by geographical location and sex. Results: In 2020, the global liver cancer incidence was estimated to be 11.6 per 100 000 persons with 905677 cases. The global HCC incidence was 8.2, with higher incidence observed in the male population (12.3 vs. 4.0 in females); Eastern Asia (21.4), Southern Europe (11.8), South-Eastern Asia (10.3), and Northern America (9.4). The global iCCA incidence was 2.7, with a higher incidence observed in the male population (3.0 vs. 2.3 in females); Eastern Asia (6.2), Northern Africa (5.5), South-Eastern Asia (4.0), and Western Europe (3.4). Conclusions: HCC remains the predominant subtype of liver cancer, especially in Eastern Asia, Southern Europe, and South-Eastern Asia. The iCCA incidence was the highest in Eastern Asia, Northern Africa, and South-Eastern Asia. Further studies are needed to explore the reasons for the disparities in liver cancer distribution by histological subtypes.
