- Abstract : Background: The Rheumatoid Arthritis (RA) is the most frequent inflammatory rheumatism. It affects 0.5 to 1% of the world's population. The osteoporosis and the vertebral fractures are well known complications of the RA. The vertebral fractures are the most common type of linked to fragility fractures. They cause chronic back pain, loss of size, kyphosis, abdominal pain, handicap and death. Objectives: Main Objective: To assess the prevalence of vertebral fractures in patients with RA VFA. Secondary Objectives: Identify potential factors vertebral fracture. Assessing the vitamin D status. Methods: We recruited 154 women with RA; none of whom were taking osteoporosis medications. Patients underwent dual X-ray absorptiometry at the hip and spine and VFA, and completed a questionnaire. The vitamin D level was assessed by measuring the level of the total 250HD. VFA was classified using a combination of Genant semi-quantitative approach and morphometry. Results: The patients had a mean (S.D) disease duration of (9.1) 6.1 years. Vertebral fractures were detected in 27% (41/154) of the subjects. This group of women showed a lower BMI and a lower BMD in the hip area. They also had a longer disease period and higher corticoid consumption. Stepwise regression analysis showed that the presence of VF was independently associated with a low hip T-score, a low 250HD level, a higher disease activity and to a longer average disease period. These results may suggest a vertebral fractures prevention program. This program would aim to rapidly treat patients by optimizing the vitamin D status of subjects with RA.
