Abstract

Abstract : Introduction: Inflammatory bowel disease (IBD) are chronic recurrent inflammatory diseases of the gastrointestinal tract, with a negative impact on patient's quality of life. Patient-reported outcome (PROs) are increasingly recognized as complementary to objective markers of disease activity, yet evidence on PROs in IBD in the real-world setting in Morocco is limited. The purpose of this study was to analyze how strong PROs correlate with clinical scores and inflammatory biomarkers for assessment of disease activity in IBD patients. Methods: A cross sectional study was conducted from July 2020 to July 2021. Adults outpatients with a confirmed diagnostic of IBD were asked to complete the IBD disk questionnaire using the validate Arabic version. The overall IBD disk score was calculated as the sum of its 10 components (regulating defecation, abdominal pain, joint pain, sleep, education and work, body image, sexual functions, emotion, energy and interpersonal interactions) ranging from 0 to 100 as the best score is 0 and the worst score is 100. The Harvey Bradshaw index (HBI) in Crohn's disease (CD) and the clinical mayo score in ulcerative colitis (UC) were determined as well as inflammatory biomarkers: Serum C-reactive protein (CRP) and fecal calprotectin (FCP) with a cutoff level > 250 microg/g to assess IBD activity. Clinical activity was defined as HBI>4 or Mayo clinical score > 1. The data was analyzed by Microsoft Excel 2020. Statistical comparisons were assessed at the 0.05 level Results: During one year study period, 308 IBD outpatients, 187 (60, 7%) patients with CD and 121 (39, 3%) patients with UC, were included. The median age was 30 (16-64) years and 47, 1% were female. The median duration of IBD was 6 years.Clinical remission was found in 48.1%.CRP and FCP were available respectively in 90, 3% and 58.8% of patients. The full completion rate of the IBD-disk was 91, 2%. The mean IBD-disk score was 43 and the median subscore from its components ranged from 2 (joint pain) to 6 (emotions), table 1. The IBD disk total score was higher for CD (score of 51 compared to 39 for UC, p=0, 03) and previous surgeries (52 vs 43, p=0, 03). There was a significant positive correlation between IBD disk total score and disease activity (CD:score of 25 compared to 56 for active disease; p<0.0001, UC: 33 vs 52; p< 0.0001) and FC (r = 0.247, p=0.001). There was a lower correlation between IBD disk total score and CRP (r= 0, 121, p=0, 04). We noted that 9, 3% of patients on clinical and biochemical remission had marked disability (31-50) with a high score in some components: body image (8), sexual functions (7) and interpersonal interactions (7). Patients with perineal disease (24, 3%) had severe disability, p=0, 003.The score showed no association with age, disease duration and gender. Conclusion: The IBD-disk score has a good completion and demonstrated a close correlation with clinical scores and biochemical biomarkers. It could be a valuable tool in routine practice.
