- Highlights: Extended periods of broad-spectrum antibiotic regimens can contribute to antibiotic resistance and ultimately limit future treatment options. The appropriateness of a single, empiric regimen for all instances of diabetic foot infections (DFIs) is difficult to determine. This review analyzes global literature relating to cultures, incidence, risk factors, resistance, and bacterial distribution in DFIs. S. aureus remains a significant pathogen, with a growing incidence of P. aeruginosa and MDR gram-negative bacilli. Local epidemiology and resistance patterns remain essential for antibiotic treatment considerations. Abstract: Diabetes mellitus continues to be an increasingly common comorbidity. Diabetic foot infections are one of the most common causes of hospitalization in this population, and account for a significant portion of increased hospitalization and healthcare expenditure. Complications, such as osteomyelitis, can necessitate the use of multiple, prolonged antibiotic courses. These courses often consist of broad-spectrum, empiric therapy determined by organisms considered to be commonly associated with these types of infections.
