Abstract

Abstract: The CHC program is expected to improve care coordination, which is hypothesized to reduce hospitalizations and improve the discharge process. We examined all-cause and ambulatory care sensitive hospitalizations (ACS), 30-day readmissions and use of post-acute care (PAC). We linked Medicaid and Medicare claims for 2013–2015 (limited to fee for service Medicare). All-cause hospitalizations declined from 498 to 471 per 1000 person-years. There were about 66 ACS hospitalizations per 1000 person-years; that rate was substantially higher among people receiving HCBS (160), compared to non-disabled duals (48). About 22% were readmitted within 30 days. The most common PAC setting was a nursing facility (30%), followed by home care (28%), 3% used inpatient rehabilitation, and 1% used long-term acute care hospital (LTACH). While people were most likely to be discharged back to the same setting, about 15% of HCBS users went to nursing facilities. These findings set the baseline for monitoring program impact.
