Abstract

Abstract: Over the past decade, settings across the health care continuum have been increasingly embracing an ideal of person-directed care which places the care recipient as the driving force in decision-making and care planning. This is straightforward when patients and residents have the cognitive ability to make decisions, as they can specify their preferences. It has been challenging for facilities to implement this model to meet the needs of people who experience varied physical, cognitive, and behavioral symptoms of dementia. In this session, we will describe three models of person-directed, individualized care delivery and dementia-capability being used in various settings, highlighting common approaches and translatability. Two presentations focus on organizational level initiatives. First, in the nursing home setting, Reinhardt and colleagues present a retrospective comparison of small house communities (organizational redesign, adoption of person-directed care processes) with traditional, legacy nursing home communities. Wyman and colleagues describe an innovative 6-component model of "Dementia Friendly Hospital" implemented across a large Veterans Affairs medical center. The last presentation addresses a provider level initiative. Gilmore-Bykovskyi and colleagues describe a nurse-driven multi-component decision support intervention, the PROACTIVE Treatment Algorithm for improving individualized care delivery management of neuropsychiatric symptoms in an inpatient medical unit. Person-directed care approaches include attention to the diverse contexts within which they are implemented. Both organizational promotion of dementia-responsive environments and direct care approaches provide opportunities and challenges for person-directed care delivery. The Discussant will highlight the complementary nature of these approaches as they span the continuum of care for persons with dementia.
