- Abstract: Objectives: To understand the association between opioid prescription dosage and hip fracture among patients 65 years and older in the community. Design: Retrospective, propensity-matched case control study Setting: Primary care clinics Participants: Patients aged 65 years and older who are empaneled in Mayo Clinic's Employee and Community Health practice from 2014–2017. There were 142 cases that experienced a hip fracture and 284 matched controls that did not have a hip fracture. Measurements: Baseline patient characteristics were modeled independently using univariate logistic regression. Conditional logistic regression models were used to investigate associations between the exposure of an opioid prescription and experiencing a hip fracture, while adjusting for patient characteristics and propensity score. Results: Around 35% of patients received an opioid prescription (35.2% of cases and 35.6% of controls). Patients who had experienced a hip fracture were more likely to have had an opioid prescription in the 30 days prior, with patients receiving more than 420 Morphine Milligram Equivalents being four times more likely to be diagnosed with hip fracture within 30 days, compared to those who did not have an opioid prescription (adjusted OR: 4.5; 95% CI 1.2- 17.8; p-value=0.03). Conclusion: Patients on higher morphine equivalent doses have increasing odds of hip fractures compared to patients without opioid prescriptions.
