Abstract

Abstract: With increasing age comes increased prevalence of urologic dysfunction and symptoms, yet current therapeutic interventions are often inadequate. The incompletely understood linkage between system, organ, and tissue domains across the lifespan remains as a knowledge gap. We hypothesized that parallel changes consistent with diminishing bladder volume sensitivity, yet preservation of voiding strength, would be observed in all three domains with aging. Voiding spot assays, pressure/flow cystometry, and bladder strip studies were performed to assess cortical input, autonomic input, and functional changes at the molecular level, respectively. Aging was associated with increased frequency of spots and decreased volume per spot in voiding pattern behavior, loss of bladder volume sensitivity and preservation of detrusor voiding strength, and no significant differences in adrenergic, cholinergic, or field-stimulation sensitivity. Our findings therefore suggest that central controls are a more powerful determinant of the aging bladder phenotype than tissue changes. Symptoms therefore can be interpreted as the result of increased chance of adaptive failure to the physiologic stress of bladder filling.
