Abstract

Abstract: Background & Purpose: Older patients are at high risk for being diagnosed with cancer. Yet, many older patients lack health information and are unprepared to partner with their doctors to discuss cancer prevention and screening during visits. This study tested an innovative educational intervention (Speak Up) to improve older adults' initiative and communication skills with primary care doctors. Methods: The "Speak Up" intervention is a 90-minute group program designed to empower older adults to take initiative in discussions about cancer screening and prevention. It consists of an educational movie, group discussion and review of helpful resource materials. It was delivered to 383 community dwelling older adults (Mean Age:75.5) attending AAA senior center programs in New York, Ohio, and Florida. We report findings based on responses to pre- and 2-month post intervention questionnaires. Results: After the intervention, participants reported significantly increased levels of confidence (t =-2.04, p-value =0.042) and preparedness during medical visits. (t = - 1.92, p-value =0.055). Over 80% of the participants also reported that the program taught them to prepare a list of medications, obtain copies of test results and ask questions at their next visit to the doctor. Additionally, over two thirds of respondents made plans to use the information they learned to discuss cancer screening with their doctor. There were no significant increases within the attention control group. Conclusions: This study suggests the preliminary efficacy of a short educational program for enhancing communication skills of older adults with their doctors regarding cancer screening and prevention.
