Abstract

Abstract : Background: The evolution of rheumatoid arthritis is associated with the progression of functional disorders. There are some factors, which influence on this process. Diseases activity is the main of the factors. Objectives: To study the influence of the level of activity on functional status in patients with early RA for the past five years of observations. Methods: We used the data of many years of the Russian prospective investigation «RADICAL» (Rheumatoid Arthritis: DIagnostic, Clinical, Aggressive Line of Treatment). In the study included 200 patients with early RA in the period from 2003 to 2007 (67.5% RF (+); 86, 5% women, age of 49 [40-58]. Duration of the disease at the time was < 2 years. The diagnosis of RA was established on basis of ACR criteria (1987). Patients were treated with DMARDs, including biologic therapy (24%), according the «tight-control» principle. Results: With the passage of time have observed a decrease in the level of disease activity. Baseline DAS28 median [25 th percentile, 75 th percentile] was 5, 1 [4, 49-5, 85]. By the fifth year of observation was 3, 12 [1, 86-4, 32]. The same trend applies to HAQ: 1, 125 [0, 625-1, 75], 0, 75 [0, 125-1, 125], respectively. In the period from 1 to 4 years of follow-up was disease stabilization. However, the 5-year observation of the increase in the values of the DAS28 and HAQ. Conclusions: DAS28 was positively associated with HAQ at every time point during 5 years. These results indicate, if the DAS28 is lowered, patients will benefit with better functional ability. This correlation confirms the importance of the timely suppression of disease activity in order to prevent the development of functional disorders in patients with early RA.
