- Abstract: Human errors occur for multiple reasons; therefore, multiple preventative actions are needed to avoid different types of error. It is necessary to avoid inadvertently inducing errors or delivering latent conditions (traps) in the workplace that can result in accidents. After reviewing both Reason's and Rasmussen's human error theories, the importance of intervening in the process design to make it difficult for the error occurs stands out. Lack of focus on Human Factors in designing the workplace and/or human machines interfaces can lead to severe losses associated with major accidents.
