- Abstract : Introduction/Background: The aim of the study was to compare the effectiveness of neoadjuvant chemotherapy regimens (NACT): Mitomycin and Cisplatin (MP) vs Paclitaxel and Cisplatin/Carboplatin (TCb/TP) in advanced BRCA1-mutated ovarian cancer (BMOC). Methodology: In a prospective cohort of 13 patients (38–66 years (mean age 52 years) we analyzed the efficacy of Mitomycin (10 mg/m 2 ) and Cisplatin (100 mg/m 2 ) NACT administered intravenously every 3–4 weeks. The control group included 22 patients of retrospective cohort (38–67 years (mean age 51, 9 years), who received standard 1-st line NACT (TCb/TP). Clinical efficacy was evaluated according to RECIST 1.1, histopathologic response - according to the Bohm scale (2015). Results: The objective response was observed in 13/13 (100%) patientsin MP group vs 9/22 (41%) patientsin TCb/TP group (p=0, 0006). Complete surgical cytoreduction was achieved in all patients (13/13; 100%) after MP vs 13/22 (59%) cases in TCb/TP group (p=0, 01). Complete pathological response was confirmed only in MP group - in 3 out of 13 patients (23%) vs 0/22 patients (0%) in TCb/TP group (p=0, 04). Conclusion: The short-term results of the present study showed that the neoadjuvant chemotherapy with Cisplatin and Mitomycin is an effective option for BRCA1-associated advanced ovarian cancer treatment.
