- Abstract : Background: Lower extremity overuse injuries are common in athletes participating in sports with repetitive jump-landings. Objective: To systematically review the evidence for kinematic risk factors during a landing task for the development of lower extremity overuse injuries in a physically active population. Design: Systematic review and meta-analysis considering prospective cohort, cross sectional or case-control study designs. Setting: Electronic databases PubMed, Embase and Web of Science were consulted in March 2019. Participants : Articles including an athletic or physically active, healthy population or with a recent/previous overuse injury of the lower extremity. Assessment of risk factors : Two reviewers independently screened for relevant studies based on title, abstract and full text. Methodological quality was assessed by a modified Downs and Black checklist. Main outcome measurements: Articles reporting on 3D lower extremity and/or full body kinematic risk factors for overuse injuries in patient groups versus controls. Results: Nineteen studies with knee overuse injuries, 3 with exertional medial tibial pain (EMTP) and 1 with groin injuries met the inclusion criteria. Overall methodological quality was moderate across studies. This systematic review exhibits evidence for decreased peak knee flexion, increased knee adduction range of motion (ROM) and increased peak hip internal rotation as risk factors for knee overuse injuries. Meta-analysis confirmed significantly increased knee internal rotation at initial contact (p<0.001) and peak knee external rotation (p=0.05) in subjects with knee overuse injuries. Conclusion: This systematic review and meta-analysis provides preliminary evidence for a stiffer sagittal landing pattern and increased accessory movements during landing as potential risk factors for lower extremity overuse injuries. Future high quality prospective studies investigating full-body 3D landing kinematics are needed. … (more)
