- Abstract : Tunneled catheters are an alternative means of vascular access for hemodialysis. Previous studies have claimed adequate urea reduction ratio (URR) associated with good blood flow rates and adequate dialysis. We examined one month data on patients in a single outpatient dialysis facility and compared adequacy of dialysis (measured by URR) in subjects with catheters to those who had grafts or fistula. A total of 73 patients were included; 51 with PA (permanent access) and 22 with cuffed venous catheter (CVC). 2 patients in the CVC group and 6 patients in PA had missing data points and therefore were excluded. Adequacy data are summarized in the following table : CVCs had a significantly lower URR compared to PA. The time on dialysis was not different in the 2 groups; lower URR was explained by lower blood flow of these catheters. CVCs are associated with suboptimal URR even in the presence of blood flow rates of 350 mL/min.
