- Abstract : Background: Premenstrual syndrome (PMS) encompasses a range of biopsychosocial symptoms, which impair normal functioning prior to menstruation and in the physically active may inhibit athletic performance. Objective: To assess the prevalence burden of PMS in a physically active female population. Design: Cross-sectional study conducted via a web-based questionnaire. Setting: Web-based questionnaire distributed through sporting clubs and organisations by direct contact and through various social media platforms. Patients (or Participants): Females 18 years of age and older, engaged in regular physical activity from recreational to elite level. Interventions (or assessment of risk factors): Participants completed an online survey, incorporating previously validated questionnaires, along with demographic and performance-related questions. Main outcome measurements: Identification of the prevalence and severity of PMS in the survey population, while highlighting measures taken by participants to reduce the impact of PMS on their physical performance. Results: 533 participants (average age 30 years) completed the questionnaire. 66.8% were participating in a competitive sport at club-level or higher. A 48.2% prevalence of PMS was identified in the study population. 10.3% of participants were taking the oral contraceptive pill (OCP) to reduce pre-menstrual symptoms, with 8.6% using the OCP to minimise impact on sporting performance. Participation in physical activity was reduced during every menstrual cycle in 5.4%, with 35.7% reducing participation during some menstrual cycles. Reasons cited for reduced participation included excessive pain (68%), menorrhagia (48.1%) and nausea (33.2%). A major inhibitory effect of menstrual symptoms on training (3%) and competition (3.6%) was reported by some athletes. Conclusions: These preliminary findings identify a higher burden of PMS than reported in previous studies. Athletes should be encouraged to report symptoms related to menstruation to reduce time missed related to training and competition. Effective management strategies should be sought for PMS symptoms in athletes that emphasise long-term health and performance. … (more)
