Abstract

Abstract : Background: In previous studies we noted that maximum bone loss around total hip prosthesis occurs in first six months after surgery. After six months bone loss becomes slower and in some cases BMD even can increase. Objectives: To study the 15-year follow up changes of BMD in patients with RA. Methods: 282 RA patients were examined at the baseline. At the end of investigation we analyzed 43 patients. DEXA was used for BMD assessment around stem (7 ROIs) and cup (3 ROIs) of prosthesis. First examination was conducted 2 weeks after the operation and repeated at 3, 5, 10 and 15 year after surgery. Results: 3 years after decrease of BMD was -5, 8% and -4, 9% around stem and cup respectively (p<0, 01). By 5 and 10 year the bone loss was not significant (-6, 1% and -5, 5%; -7, 1% and -6, 3% around stem and cup respectively). 15 years after surgery we observed significant increase of bone loss (-18, 9% and -22, 7% around stem and cup respectively (p<0, 01)) Conclusions: These data supports our opinion of stress remodeling of bone around hip prosthesis. This phenomenon doesn't stop after 1 year but keep on even 15 years after surgery.
