Abstract

Abstract : Problem statement: Annual reviews for children with complex needs in community paediatrics are variable in quality and consistency. These children have chronic conditions, functional limitations, increased health and other care costs. This results in uncertainty and potentially avoidable patient harm. Aim: To improve quality, consistency and efficiency of annual reviews for children with complex needs. Methods/measures: We included children with complex needs from our community paediatrics clinic from March to September 2018. We excluded children with Down's syndrome or autism since they have a pre-defined care pathway. Process maps and fishbone diagrams identified contributing factors and possible areas for intervention, which were narrowed down to reducing preparation time and standardising content of annual reviews. We measured the content of annual reviews from analysis of consistency and content of clinic letters. We obtained qualitative feedback from doctors, secretaries and patients. Change idea: Co-Design and implement an annual review proforma for children with complex needs with input from consultants, secretaries and patients. Conclusions: An annual review proforma for children with complex needs can be helpful for standardising and streamlining their annual reviews. Quality improvement strategies using the model for improvement (Table 1 ), can be easily applied to improving care in community paediatrics.
