- Key Points: The transfemoral approach is used in most patients undergoing transcatheter aortic valve replacement (TAVR) and is associated with low mortality rates as compared with alternative access sites. The technical aspects of the procedure rather than the patient's risk profile explain most of the differences in mortality rates observed across different access routes. Future TAVR studies are needed to establish the optimal access site when the ilio‐femoral artery anatomy is unfavorable.
