- Abstract : EVIDENCE‐BASED ANSWER : In acute sinusitis, intranasal steroids resolve or improve symptoms slightly better than placebo, with higher doses more effective than lower doses (SOR: A, meta‐analysis of RCTs). In acute rhinosinusitis, fluticasone intranasal spray once or twice daily improves symptom scores slightly more than placebo (SOR: B, single RCT). Intranasal steroids are an option for viral rhinosinusitis and acute bacterial rhinosinusitis and recommended for chronic rhinosinusitis (SOR: B, evidence‐based guideline).
