- Prevention of cardiovascular complications of hypertension has become a hallmark of the therapeutic approach to disease management. Among the many facets of the problem, the association between noninsulin-dependent diabetes mellitus, hypertension and renal function has emerged as a major risk factor for the development of end-stage renal disease requiring dialysis or renal transplantation. This paper reviews the promisory outcomes of long-term clinical trials that demonstrate a renoprotective effect of angiotensin II antagonists on the progression to end-stage renal disease in patients with diabetic nephropathy. The lessons learned from the publication of these trials provide a positive new approach to risk reduction and prevention of loss of renal function in this population.
