- Abstract: Background: Fibromyalgia is a chronic pain syndrome characterised by widespread diffuse pain and multiple tender points along with sleep disturbances, fatigue, and psychological distress. While a systematic review has shown that fibromyalgia patients had higher serum levels of interleukin-1 receptor antagonist (IL-1Ra), interleukin-6 (IL-6), and interleukin-8 (IL-8); Indian data is scanty. The aim of this study was to analyse serum cytokines pattern in an Indian population and its correlation with disease severity. Methods: This was a prospective case-control study conducted at tertiary care teaching hospital in South India. 21 females fulfilling the 2016 modification of the 2010/11 ACR fibromyalgia criteria and not having co-existent major depressive disease or inflammatory diseases were studied. They were compared with 20 age and sex-matched controls. The outcome measures used to study disease severity were Visual Analogue Score (VAS) Pain, Fibromyalgia Impact Questionnaire - Revised (FIQR), BPI Pain Scores and SF-36 Health Survey. Serum cytokine assays (IL-6, IL-8 and Il-1Ra) were done using ELISA. The statistical testing was done using student's t test, Mann-Whitney's test, Pearson's correlation, Spearman rank correlation and Chi square tests. Results: 21 female patients and 20 controls were matched for age, sex, BMI and co-morbid conditions were studied. The mean disease duration (mean + S.D) of patients was 6.47 + 5.47 years. The outcome measures (mean + S.D) are as follows: - VAS Pain (76.67 + 16.68), BPI-Average Pain (761.86 + 150.30), BPI-Function (636.44 + 198.88), FIQR (59.90 + 13.63), SF36- Physical Component Summary (21.47 + 7.55), SF36-Mental Component Summary (29.11 + 10.38). The serum IL-1Ra levels were significantly lower in fibromyalgia patients (393.77 + 347.025) than control group (687.02 + 370.15) (p = 0.019). However, there is no statistical difference in the levels of IL-8 and IL- 6 between patients and control. Analysis for correlation between cytokine and outcomes measures, showed a moderate positive correlation between serum IL-8 level and VAS-Pain score (p = 0.047) and a borderline positive correlation between serum IL-8 levels and BPI average pain scores (p = 0.059). Conclusion: The cytokine profile in an Indian cohort of fibromyalgia is different from the western population. The serum IL-1Ra levels in fibromyalgia patients are significantly lower than controls in our cohort. Also, serum IL-8 levels tend to correlate with the pain scores (VAS pain and BPI average pain). The cytokine profile in fibromyalgia is heterogenous across different populations and thus other cytokines also need to be studied more. Disclosures: S. Surendran None. M. C.b. None. … (more)
