- Abstract: Introduction: Sleep misperception is a common and well-known condition that can influence sleep studies. Few evidences addressed the frequency and associated factors of the entire sleep misperception spectrum. We aimed to evaluate the frequency and predictive factors of individuals with accurate sleep estimation, underestimators, and overestimators in a representative sample of Sao Paulo city, Brazil. Methods: This was a cross-sectional investigation with the data provided by EPISONO cohort. A total sample of 971 participants of 20–80 years-old were included in the study. Sleep misperception classification as accurate, underestimators, and overestimators were performed according to the subjective sleep ratio values. Sleep, clinical, psychological, and biochemical parameters were compared among the groups. Results: A mean objective total sleep time of 342 ± 2.4 min and subjective of 391.6 ± 2.9 min were observed, with a sleep discrepancy of 4.8 min and a misperception index of -0.04 min. Underestimators and overestimators composed 28.8% and 35.6% of the sample, respectively. Early morning awakening insomnia was associated with a 2.67-fold increased risk of negative sleep misperception. C-reactive protein level predicted a 2-fold increased risk of overestimation of sleep time. Conclusion: Different spectra of sleep misperception show distinct associations with sleep and biochemical parameters.
