- Abstract: Introduction: Numerous epidemiological studies have identified an increase in body weight, commonly reported as body mass index (BMI), as a strong risk factor for developing, and increasing the severity of, obstructive sleep apnea (OSA). Recent research suggests that BMI may not be the gold standard measurement to predict OSA severity, and that other measures may be more predictive. This project examines whether other anthropomorphic measures (waist, neck, and hip circumference) may be better predictors of OSA severity. Methods: Men (47%) and women (53%) Sleep Heart Health Study (SHHS) participants aged 40+ (n = 5237) were recruited from cohort studies already in progress where data on several cardiovascular disease risk factors, as well as follow-up and outcome information, was already collected. SHHS participants underwent overnight polysomnography to assess for OSA and other sleep disorder breathing (SDB) issues. Participants also completed additional self-report questionnaires on symptoms of sleep disturbances. Stepwise linear regression analysis was conducted using OSA severity as the outcome variable and waist, neck, and hip circumference, along with BMI, as explanatory variables. Results: A significant regression model F (6, 415) = 21.35, p < 0.01, explained 61% of the variance in OSA severity (r 2 =0.61). Significant predictors were neck circumference (β = .46, p < 0.01) and waist circumference (β = .38, p < 0.01). The explanatory variables of hip circumference and BMI were not significant in the model (p = .064 and p = .052, respectively). Conclusion: Although the alternate anthropomorphic measures of waist, hip, and neck circumference may be inter-related with BMI, these results suggest that such measures may be better associated with predicting OSA severity than BMI. Hip circumference may not be a significant predictor of OSA severity due to its inherent gender differences. It will be important for further research to determine how other covariates factored into regression models may affect the significance of an anthropomorphic measure on predicting the severity of OSA. The clinical significance of using alternate anthropomorphic measures should be investigated in future studies.
