Abstract

Abstract: Malnutrition affects up to 60% of children undergoing cancer treatment and is associated with impaired response to treatment and outcome of disease. When patients cannot meet nutritional needs orally, evidence-based practice supports the use of enteral over parenteral nutrition when tolerated. However, implementation of enteral nutrition can be difficult due to inconsistent involvement of dietitians, inconsistent approach to nutrition by health care providers (HCPs), and variable attitudes/beliefs among providers and families. The aim of this project was to standardize the approach of HCPs to nutrition support by providing a standardized algorithm and educational approach. The algorithm included a consult with dietitians at diagnosis, as well as specific criteria about when and for how long to trial oral supplements and appetite stimulants before initiating enteral nutrition. The project also included educational materials (handouts and a video describing the overall benefits of nutrition support, types of nutrition support, and patient/caregiver testimonials). Participants included 10 CNS/solid tumor and 10 leukemia/lymphoma patients and their caregivers. Dyads completed surveys 6-months post-diagnosis to assess perception of nutrition support and the education provided. One patient expired prior to completion of survey; an additional 2 patients are not yet 6-months post-diagnosis. In examining the use of the algorithm, all patients saw a dietitian within the prescribed timeframe, 75% received the nutrition handout, 80% were started on an appetite stimulant, and 40% had a nasogastric tube placed. Only 2 of the 8 eligible patients watched the video. Full findings will be included in the final presentation.
