- Abstract: Introduction: Lower REM theta (4-7Hz) power spectral density (PSD) has been observed in trauma-exposed persons who developed Posttraumatic Stress Disorder (PTSD) compared with those who did not. Thus greater REM theta PSD may be associated with resilience following trauma. PTSD is often accompanied by insomnia, both disorders share hyperarousal symptoms, and pre-trauma insomnia increases risk of post-trauma PTSD. If persons with Primary Insomnia (PI) show lowered REM-theta, this might be associated with increased risk for PTSD. We therefore compared REM theta PSD between persons with PI to good sleepers (GS). Methods: Following an acclimation night of ambulatory PSG (Somte-PSG, Compumedics), 10 PI (9 Female, Age: 34, SD: 16.51) and 12 GS (8 Female, Age: 31.59, SD: 14.03) completed 2 consecutive nights of PSG before (Night2) and after (Night3) undergoing a validated fear conditioning and extinction procedure during fMRI. Signal filtration of EEG data was processed using an in-house Matlab program written by author KK using the EEGLAB toolbox (http://sccn.ucsd.edu/ee g lab/). To compute theta PSD, Matlab's PSD and band-power functions were used (Hanning window 512 samples, 2 seconds). Absolute and relative theta PSD for frontal (F3, F4) and central (C3, C4) derivations were then averaged for each artifact-free 30-sec REM epoch. Results: For REM theta absolute power we observed a Group main effect trend [F(1, 19)=3.58, p=0.074] in frontal derivations. For REM theta relative power we observed a Group main effect trend in frontal derivations [F(1, 20)=3.62, p=0.072] and a significant Group main effect in central derivations [F(1, 19)=7.26, p=0.014]. There were no main effects or interactions of Night or Hemisphere nor were there any interactions with Group. Conclusion: This analysis reveals a tendency for REM theta PSD to be lower in PI vs. GS.
