- Abstract: Introduction: Children with congenital heart disease (CHD) are known to have increased risk of neurocognitive impairment. Prior research has shown an increased risk of sleep-disordered breathing (SDB) in children with CHD. Given that SDB is a known cause of neurocognitive impairment in children without CHD, we hypothesized that SDB may contribute to the observed neurocognitive impairment in children with CHD. Methods: Children age 6–17 years of age with corrected CHD underwent home sleep testing and neurocognitive testing including an IQ test (Kaufman Brief Intelligence Test) as well as validated tests of memory (paired associates learning test, PAL) and attention (simple reaction test, SRT) from the Cantab neurocognitive testing battery. Children with an obstructive apnea-hypopnea index (AHI) >1 or central AHI >5 were considered to have SDB. Independent t-tests were used to determine significance. Results: Complete results were available for 32 children. Eighteen children (56%) were found to have SDB, which was obstructive sleep apnea in all cases. Total IQ was markedly lower in children with CHD and comorbid SDB compared to children with CHD without comorbid SDB (mean 98 ± 11, vs 85 ± 13, p=0.007). Children with CHD and SDB had more recall errors in the PAL than children with CHD without SDB (mean 5.4 ± 5.1 vs 1.7 ± 2.3, p=0.016). Similarly, children with CHD and SDB tended towards worse performance on the SRT. Conclusion: Children with CHD and comorbid SDB appear to have impaired neurocognition compared to children with CHD without comorbid SDB. SDB may be a reversible cause of neurocognitive impairment in children with CHD. Further research is needed to confirm this finding, as well as evaluate the effects of SDB treatment on neurocognitive impairment in children with CHD.
