- Abstract: Introduction: Few studies have investigated the validity of obstructive sleep apnea (OSA) screening questionnaires in general population. The aim of the present study was to assess the diagnostic value of three OSA screening questionnaires (the Berlin Questionnaire, BQ; the STOP-Bang Questionnaire, STOP-B; the Four-Variable Screening Tool, Four-V) in a Korean community sample. Methods: A total of 1148 community-dwelling participants who were selected based on the population statistics of South Korea completed the BQ, the STOP-B and the Four-V. Among them, an overnight in-laboratory polysomnography (PSG) was conducted in 116 participants randomly selected in consideration of age, sex and risk group classification of each screening questionnaire. Apnea-hypopnea index (AHI) ≥ 5 was considered as OSA, and AHI ≥ 15 was considered as moderate to severe OSA. Sensitivity, specificity, positive predictive value, negative predictive value, positive likelihood ratio, negative likelihood ratio and area under curve (AUC) were calculated. Results: The Four-V with cutoff ≥ 8 showed the highest sensitivity (69.4%), followed by the BQ, the STOP-B (61.3%, both) and the Four-V with cutoff ≥ 9 in predicting OSA. However, the sensitivity of the STOP-B was higher (72.4%) than the Four-V (68.5%), the BQ (63.0%) and the Four-V with cutoff ≥ 9 to predict moderate to severe OSA. The STOP-B also had the largest AUC (0.75; 95% CI, 0.66–0.84) than Four-V (0.72; 95% CI, 0.62–0.81) and BQ (0.62; 95% CI, 0.52–0.72). Conclusion: The sensitivities of three screening questionnaires for detecting OSA in a community sample are lower than those reported in clinical settings, but the specificities were relatively high.
