- Abstract: Introduction: Disruption in regular menstrual cycle could modulate inflammatory profile. This context indicates that anovulation status could be related to low-grade inflammation leading to health impairments. We hypothesized that anovulatory women had higher circulating levels of inflammatory markers compared with women in regular menstrual cycle or in use of contraceptives, and this proinflammatory condition was associated with changes in sleep pattern. Methods: A subset sample from the Sao Paulo Epidemiologic Sleep Study - EPISONO was sellected. Women in reproductive age were distributed in 3 groups: regular menstrual cycle (195), hormonal contraceptive use (76) and anovulatory menstrual cycle (21). Subjects underwent a whole-night polysomnography, answered questionnaires, and had their blood collected for inflammatory (C-reactive protein, tumor necrosis factor-alfa, interleukin-6), cardiovascular (total cholesterol, high-density lipoprotein, triglycerides, homocysteine) and hormonal (follicle-stimulating hormone, luteinizing hormone, progesterone, estradiol, free and total testosterone, insulin, glucose) analysis. Results: Women with anovulatory menstrual cycle had higher circulating levels of the proinflammatory cytokine interleukin-6 (IL-6) compared with regular menstrual cycle and hormonal contraceptive groups. No other classical marker of low-grade inflammation was significantly different. The analysis of associated variables of the inflammatory profile demonstrated that SpO2 mean during sleep was a predictive factor of IL-6 levels. Conclusion: Our data suggests that in premenopausal women with anovulation a proinflammatory condition mediated by IL-6 is associated with oxygen levels during sleep. These findings reflect the balance between gynecological status, immune system, and sleep, pointing to the need to control these factors in clinical practice and research contexts.
