- Abstract : Objective: Long standing hyperuricemia induced the proinflammatory cytokine activity and others mediator inflammation. Macrophage and neutrophil were the main cell producing inflammatory mediator such as IL-1, TNF-a, IL-6, IL-8 and GM-CSF (Granulocyte-Macrophage Colony-Stimulating Factor). Johnson RJ et all, demonstrated that the urate penetrate into the vascular smooth muscle cell and caused cell proliferation, activated renin-angiotensin system, and stimulated production of various inflammatory mediators including CRP and MCP-1. Design and Method: This research was randomized control trial at outpatient clinic and wards at Internal Medicine Department of Mohammad Hoesin Hospital in Palembang since February until August 2014. Subjects were hypertension patient with hyperuricemia divided to allopurinol group who received 100 mg allopurinol twice a day and placebo group received saccharin twice a day for one month. Results: Variable inflammation, such as LED mean level was 19 mm/hour, WBC 8200 /mm 3, median level of CRP 12, 72 mg/dl in allopurinol group and 11, 50 mg/dl in placebo. After a month drug admission (allopurinol and placebo), the CRP median level were decreased (5, 27 mg/dl) in allopurinol group a slight decreased of median level of CRP (10, 78 mg/dl) in placebo group. Statistical test demonstrate significant effect of allopurinol to reduce the CRP level (p = 0, 03).
