Abstract

Abstract : Objective: To study association between aortic stiffness and intradialytic hypertension in maintenance hemodialysis patients. Design and Method: A cross sectional study was used to evaluate the association between intradialytic hypertension and aortic stiffness. Intradialytic hypertension is defined as every 10 mmHg increase in systolic blood pressure (SBP) during dialysis. The parameters of aortic stiffness were carotid-femoral pulse wave velocities (cf-PWV), central pulse pressure (PP), augmentation index (AI), and augmentation pressure (AP). These parameters were measured using sphygmocor or tonometry applanation. The laboratory values were based on last routine examination. All baseline characteristic data were presented in descriptive table. The difference of mean cf-PWV, PP, AP, and AI to the presence of intradialytic hypertension were analysed by independent T-Test Results: We included 106 hemodialysis (HD) patients with the minimum duration of HD of 3 months. Mean of age was 51, 15+13, 7 years old. Fifty patients (47, 16%) had intradialytic hypertension. There were no significant difference in age, dialysis vintage, and comorbidities between the two groups. There were significant differences in haemoglobin level (10, 20+2, 01 vs 9, 39+1, 87; p 0, 035) and serum calcium level (8, 91+1, 69 vs 8, 04+1, 35; p 0, 04) but not in phosphate level (5, 96+2, 00 vs 5, 79+1, 85; p 0, 713) between the two groups. The cf-PWV, PP, and AP were significantly higher in the group with intradialytic hypertension (cf-PWV 9, 06+1, 49 m/s, PP 49, 92+19, 04 and AP 10, 42+10, 78) compared to the group without intradialytic hypertension (cf-PWV 7, 69+1, 492 m/s, PP 38, 95+12, 24 and AP 6, 13+8, 023) with p value of 0, 000, 0, 003 and 0, 021 for cf-PWV, PP, and AP, respectively. We found no significant difference in AI. Conclusions: Aortic stiffness associated to intradialytic hypertension in maintenance hemodialysis patients.
