Abstract

Abstract : Background: Reports of canine pyoderma gangrenosum (PG) are uncommon in the veterinary literature. Rarer still are cases describing dogs with both skin lesions and internal organ involvement. Objective: To describe a case of canine PG with skin and internal organ involvement. Animals: A client‐owned dog. Methods and materials: Complete blood count, serum chemistry, C‐reactive protein and SNAP cPL tests, and abdominal ultrasonography and fine‐needle aspiration of the spleen were performed. Results: The dog was treated with oral prednisolone and ciclosporin. After three months of therapy, ultrasonography revealed normalization of the spleen and resolution of skin lesions. Conclusion and clinical importance: Dogs with skin lesions compatible with PG should be screened carefully for internal organ involvement. Ciclosporin may be a useful treatment for the immediate and long‐term management of canine PG. Résumé: Contexte: Les descriptions de pyodermite canine gangréneuse (PG) sont rares dans la littérature vétérinaire. Plus rares encore sont les cas décrivant les chiens avec à la fois des lésions cutanées et une atteinte d'organes internes. Objectif: Décrire un cas de PG canine avec atteinte cutanée et des organes internes. Sujets: Un chien de propriétaire. Matériel et méthode: Une numération formule, une biochimie, une protéine C réactive, un SNAP‐test cPI, une échographie abdominale et une aspiration à l'aiguille fine de la rate ont été réalisés. Résultats: Le chien a été traité avec la prednisolone orale et de la ciclosporine. Après trois mois de traitement, une échographie a révélé une normalisation de la rate et une résolution des lésions cutanées. Conclusion et importance clinique: Les chiens avec lésions cutanées compatibles avec PG devraient être examinés avec attention pour l'atteinte des organes internes. La ciclosporine peut être un traitement utile pour la gestion immédiate et au long cours de PG canine. Resumen: Introducción: publicaciones veterinarias acerca de la pioderma gangrenosum canina (PG) son poco frecuentes en la literatura. Aún más raros son los casos que describen perros con lesiones en la piel y con afectación de órganos internos. Objetivo: describir un caso de PG canina con afectación de la piel y de los órganos internos. Animales: un perro de un propietario particular. Métodos y materiales: se realizó hemograma completo, química del suero, pruebas de proteína C reactiva y SNAP cPL, y ecografía abdominal y aspiración con aguja fina del bazo. Resultados: el perro fue tratado con prednisolona oral y ciclosporina. Después de tres meses de tratamiento, la ecografía reveló la normalización del bazo y la resolución de las lesiones cutáneas. Conclusión e importancia clínica: los perros con lesiones cutáneas compatibles con PG deben examinarse cuidadosamente para detectar la afectación de órganos internos. La ciclosporina puede ser un tratamiento útil para el manejo a corto y a largo plazo de la PG canina. Zusammenfassung: Hintergrund: Berichte über eine Pyoderma gangrenosum (PG) des Hundes kommen in der Veterinärliteratur selten vor. Noch seltener kommen Fälle bei Hunden vor, die sowohl eine Hauterkrankung wie auch eine Beteiligung der inneren Organe beschreiben. Ziele: Die Beschreibung eines Falles von caniner PG mit Hautbeteiligung und Beteiligung der inneren Organe. Tiere: Ein Hund in Privatbesitz. Methoden und Materialien: Komplettes Blutbild, Serumbiochemie, C‐reaktives Protein und SNAP cPL Tests, ein Abdomen Ultraschall und ein Feinnadelaspirat der Milz wurden durchgeführt. Ergebnisse: Der Hund wurde mit Prednisolon per os und mit Ciclosporin behandelt. Nach einer drei monatigen Therapie zeigten sich bei der Ultraschalluntersuchung eine normale Milz und abgeheilte Hautveränderungen. Schlussfolgerung und klinische Bedeutung: Hunde mit Hautveränderungen, die mit PG vergleichbar sind, sollten genau auf eine Beteiligung der inneren Organe untersucht werden. Bei Ciclosporin könnte es sich um eine nützliche Behandlung für ein unmittelbares sowie für ein langfristiges Management von PG des Hundes handeln. 要約: 背景: 犬のpyoderma gangrenosum(PG)の報告は獣医学文献では一般的ではない。いまだ皮膚病変と内臓病変を伴う犬を記述した症例はより珍しい。 目的: 本研究の目的は、皮膚および内臓病変を伴う犬のPG症例を記述することである。 被験動物: 1頭の飼育犬。 材料および方法: 全血球計算、血清化学、C反応性タンパク質およびSNAP cPL検査、ならびに腹部超音波検査および脾臓の穿刺吸引を実施した。 結果: 犬を経口プレドニゾロンおよびシクロスポリンで治療した。 治療開始から3ヵ月後、超音波検査は脾臓の正常化と皮膚病変の消散を明らかにした。 結論と臨床的重要性: PGに一致した皮膚病変を有する犬に対し、内臓病変の関与について慎重にスクリーニングすべきである。 シクロスポリンは、犬PGの即時および長期管理に有用な治療法となる可能性がある。 摘要: 背景: 在兽医学文献中, 关于犬坏疽性脓皮病(PG)的报道并不常见。而同时描述犬皮肤损伤和涉及内脏器官的病例更为罕见。 目的: 报告一例犬涉及皮肤和内脏器官的坏疽性脓皮病病例。 动物: 一只私家犬。 方法和材料: 进行全血细胞计数、血清生化、C反应蛋白、快速胰腺炎检测、腹部超声检查和脾脏细针穿刺。 结果: 该犬口服泼尼松龙和环孢菌素进行治疗。治疗三个月后, 腹部超声显示脾脏正常化, 同时皮肤病变消退。 结论和临床意义: 对于伴有坏疽性脓皮病皮肤病变的犬, 应仔细检查是否牵连内脏器官。 环孢菌素可能是治疗短期和长期性犬坏疽性脓皮病的有效方法。 Resumo: Contexto: Relatos de piodermite gangrenosa (PG) são incomuns na literatura veterinária. Ainda mais raros são os casos descrevendo cães que apresentam lesões cutâneas e envolvimento de órgãos internos concomitante. Objetivo: Descrever um caso de PG canino com envolvimento cutâneo e de órgãos internos. Animais: Um cão de proprietário. Métodos e materiais: Realizou‐se hemograma, bioquímica sérica, proteína C‐reativa, teste de SNAP cPL, ultrassonografia abdominal e aspiração por agulha fina do baço. Resultados: O cão foi tratado com prednisolona por via oral e ciclosporina. Após três meses de tratamento, a ultrassonografia revelou normalização do baço e resolução das lesões cutâneas. Conclusão e importância clínica: Em cães com lesões cutâneas compatíveis com PG deve ser feita rigorosamente a triagem para envolvimento de órgãos internos. A ciclosporina pode ser um tratamento útil no manejo da PG imediato e a longo prazo.
