Abstract

Abstract : Objective: Analyse changes of cortical excitability and plasticity in patients with Parkinson's disease (PD) according to the applied therapy using PAS 25 protocol. Material and Methods: We examined 10 patients with drug naive PD (DNPD), 10 PD patients without any complication and side effect of dopaminergic medication and 10 patients with LID. The control group (CG) consisted of 10 healthy subjects who were matched to the patients by age and sex. The excitability of the hand primary motor cortex of the more affected side were evaluated using TMS techniques. Results: Input–output curve had smaller slope in PD patients with dyskinesia's at 120% ( p = 0.040) and 130% of rMT ( p = 0.014). SP was reduced with intensity of the voluntary contraction [F(2, 35) = 88.760, p = 0.001], without group differences and without PAS effect. SICI shows significant differences between CG and DNPD at ( p = 0.010). Conclusions: All patient groups had deficits of LTP – like plasticity – diminished slope of IOC and impaired SICI in DNPD and group with dyskinesia's. LTP plasticity that has been damaged at the beginning of the disease and seems to be physiological markers for the pathological process in PD.
