- Abstract : Objective: Isolated systolic hypertension (ISH) is an uncommon type of hypertension (HT) among young hypertensives. Individuals with ISH but normal aortic (ao) systolic blood pressure (BP) have been labelled as spurious hypertensives. This form of HT has only been described among young males and has been scarcely studied. Current guidelines do not recommend starting antihypertensive drug treatment but consider periodic follow-up. The aim of this study is to evaluate vascular organ damage in relation to aortic and peripheral BP in young true hypertensives (tHT) and spurious hypertensives (sHT). Design and method: All males (aged < 40) referred because of suspicion of essential HT were included in the study. The control group consisted of healthy normotensives (NT). Office BP, 24h-ABPM (Spacelabs® 90207), aoBP by radial applanation tonometry (Sphygmocor AtCor®) and ultrasound carotid intima-media thickness (cIMT) were assessed. Patients were classified as: tHT if 24h-BP was above/equal 130 and/or above/equal 80mmHg and aoSBP above/equal 121mmHg, or sHT if 24h-systolic BP was above/equal 130mmHg and 24h-diastolic BP < 80mmHg, with aoSBP < 121mmHg. Results: n = 45 (15 NT + 30 HT). Prevalence of tHT and sHT was 70% (n = 21) and 30% (n = 9) respectively. Main parameters by group are shown in Table. Figure. As compared to normotensives, PWV shows a trend to be higher in sHT (0.289 [-0.104; 1.904, p = 0, 078]) and in tHT, (0.360 [-0.083; 1.884, p = 0, 072]). Adjusted R of the model 0, 447 p = 0.026. Conclusions: In this population: - sHT do not show statistically significant greater vascular organ damage than NT. - sHT do not seem to have greater PPA than NT. - PWV trended to be higher in sHT than in NT subjects suggesting a higher vascular damage in sHT. … (more)
