Abstract

Abstract : Objective: To assess the sleep-breathing disorders and their dynamics in patients with resistant hypertension after renal denervation. Design and method: Overall 20 patients with verified drug-resistant hypertension were enrolled in the study: 8 males and 12 females, mean age – 56 (27–67) years. All patients were overweight or obese (body mass index, BMI > 25 kg/m2). All patients underwent renal denervation according to the standard protocol. Prospective study with the complete examination was conducted monthly within six months after the intervention. Full polysomnography (Embla N7000 (MedCare Flaga, Iceland) was performed in 17 subjects at baseline and one year after the intervention. Results: At baseline office systolic blood pressure (BP) was 160 (125; 197) mm Hg and diastolic BP - 95 (71; 138) mm Hg; obstructive sleep apnea (OSA) was diagnosed in 10 (58, 8%) people: mild degree in 3 cases, moderate – in 5 patients, and 2 subjects had severe degree (X 2 = 10; p = 0, 02). Nine months after renal denervation the level of office BP [both systolic and diastolic – till 145 (100; 155) and 60 (53; 66) mm Hg, respectively], and the average daily BP (from 171.5/85 mm Hg to 141.5/72 mm Hg) significantly reduced, while BMI has increased from 29, 8 to 32, 5 kg/m2. At one-year follow-up PSG demonstrated an increase of AHI from 5.7/h to 13.3/h (p < 0.05). However, the number of central apnea episodes decreased (p < 0.05). Conclusions: Renal denervation led to a permanent long-term reduction of BP. Deterioration of sleep apnea one-year after the intervention may be associated with the increased BMI.
