- Abstract : Objective: Recent studies have shown that hyperaldosteronism in patients with primary aldosteronism (PA) is associated with vascular fibrosis which may be reflected in increased pulse wave velocity (PWV) but there is no studies that assessed the local stiffness of the carotid arteries in PA patients. To evaluate the changes of the renin-angiotensin-aldosterone system (RAAS), dynamics of blood pressure (BP) as well as stiffness of aorta and carotid arteries in patients with established diagnosis of adrenal aldosteroma before surgery and after 6 months of follow-up. Figure. No caption available. Design and method: We studied 8 patients before and 6 months after adrenalectomy for aldosterone producing adenoma (APA). The following parameters were assessed: clinic BP, plasma renin activity (PRA) and aldosterone concentration (PAC) («IMMUNOTEC», FRANCE); carotid-femoral pulse wave velocity (cfPWV) (Sphygmocor, AtCor, Australia); Distensibility Coefficient (DC), Cross-sectional compliance (CC) and Stiffness index ß (IU-22, Philips, Holland). Data were represented as median (Med) and 25%-75% interquartile range or mean and standard deviation. p-value < 0, 05 was considered as statistically significant. Results: As compared to baseline normalization of PAC, PRA and serum potassium levels were observed after adrenalectomy in all cases. All changes were statistically significant. Clinic BP also statistically significant decreased. Moreover adrenalectomy also resulted in improvment of elastic properties: CC and DC increased significant and Stiffness index ß and PWV decreased also statistically significant. The main results are shown in Table 1. Conclusions: Our results indicate that adrenalectomy in patients with APA has a beneficial effect not only on BP and RAAS parameters but also on indices of systemic arterial compliance and local carotid stiffness. It is the first time we demonstrate the associate of effective adrenalectomy in PA with positive dynamics in parameters of local carotid stiffness. … (more)
