- Abstract : Objective: To evaluate the therapeutic efficacy of a triple single-pill combination (SPC) of perindopril/indapamice/amlodipine (Triplixam®) in patients with difficult-to-control arterial hypertension (HTN). Design and method: A prospective pilot study, conducted for the period June-November 2014. Fifteen consecutive hypertensive patients on combination therapy and insufficient control of HTN were included (8 males, 53.3%), mean age 58.6 ± 7.2 years. A male patient from the initial group was subsequently excluded from the study and data analysis because of an adverse event during the first week of treatment. All patients strarted treatment with Triplixam®, including perindopril, indapamide and amlodipine in doses 5, 1.25 and 5 mg respectively - 1 tablet per day. Clinical follow-up visits were held at the end of week 2, month 1, 2 and 3 after initiation of the new treatment. Results: The mean baseline values of the systolic (SBP) and diastolic (DBP) blood pressure of the analyzed group were 165.2 ± 6.5 and 97.1 ± 4.9 mmHg respectively. The mean SBP and DBP at the end of week 2 were145.2 ± 4.3 and 88.7 ± 3.6 mmHg and at month 3 - 134.7 ± 2.6 and 84.1 ± 3.6 mmHg respectively. Most significant reduction of SBP and DBP was observed during the first 2 weeks - with 12.1 and 8.7% respectively compared to the baseline levels (p < 0.0001). Increase of the dosage of the study drug was considered for 4 patients - Triplixam® 10/2.5/5 mg - 1 tablet per day. At month 1 target SBP and DBP values were achieved by 11 (78.6%) and 13 (92.9%) patients. Further increase of the dosage of Triplixam® was considered fo 1 patient - 10/2.5/10 mg - 1 tablet per day. At month 3 all 14 patients from our study had achieved the target SBP and DBP values. An adverese event occured in 1 patient from the initial group - mild edema of the lower limbs Conclusions: The triple single-pill combination of perindopril/indapamide/amlodipine, evaluated in our study is effective, well-tolerated and safe in patients with difficult to treat HTN. … (more)
