- Abstract: Synergies between securing sexual rights and the right to health have been pursued where there are clear public health gains to be made, such as lowering incidence of HIV and other sexually transmitted infections (STI). South Africa's 1996 Constitution outlawed discrimination on the basis of sexual orientation and promoted the right to health. This qualitative health policy analysis sought to understand why and how interventions to improve sexual health of lesbian and bisexual women and address sexual violence were initially proposed in the HIV & AIDS and STI Strategic Plan for South Africa 2007-2011 and why and how these concerns were deprioritised in the National Strategic Plan (NSP) on HIV, STIs and TB 2012-2016. A conceptual framework considered several determinants of political priority for the inclusion in NSP development in 2007 and 2011 around sexual health concerns of women who have sex with women. This article presents findings from 25 in-depth key informant interviews and document review and highlights results of application of categories for a framework on determinants of political priority for lesbian and bisexual women's issues to be included in South Africa's NSP including: actor power, ideas, political context and issue characteristics. Résumé: On recherche les synergies entre la réalisation des droits sexuels et le droit à la santé si elles peuvent clairement apporter des bénéfices de santé publique, par exemple une baisse de l'incidence du VIH et d'autres infections sexuellement ransmissibles (IST). La Constitution sud-africaine de 1996 a interdit la discrimination sur la base de l'orientation sexuelle et a consacré le droit à la santé. Cette analyse qualitative des politiques de santé souhaitait comprendre pourquoi et comment les interventions destinées à améliorer la santé sexuelle des lesbiennes et bisexuelles, et s'attaquer à la violence sexuelle, étaient initialement proposées dans le Plan stratégique de lutte contre les IST et le VIH/sida pour l'Afrique du Sud 2007-2011, et pourquoi et comment ces préoccupations n'étaient plus prioritaires dans le Plan stratégique national sur le VIH, les IST et la tuberculose 2012-2016. L'article montre comment les facteurs épidémiologiques et structurels de la vulnérabilité des lesbiennes et bisexuelles au VIH et aux IST, notamment la violence sexuelle et d'autres violations de leurs droits sexuels, ont été exprimés dans des tribunes politiques et il détermine si cela a eu des conséquences sur la capacité des lesbiennes et bisexuelles à revendiquer leur droit à la santé. Resumen: Las sinergias entre asegurar los derechos sexuales y el derecho a la salud han sido buscadas cuando hay avances concretos por lograr en salud pública, tales como disminuir la incidencia del VIH y otras infecciones de transmisión sexual (ITS). La Constitución de 1996 de Sudáfrica ilegalizó la discriminación basada en orientación sexual y promovió el derecho a la salud. Este análisis cualitativo de políticas de salud buscó entender por qué y cómo intervenciones para mejorar la salud sexual de mujeres lesbianas y bisexuales y abordar la violencia sexual fueron propuestas inicialmente en el Plan Estratégico sobre VIH/SIDA e ITS para Sudáfrica 2007-2011, y por qué y cómo se restó prioridad a estas inquietudes en el Plan Estratégico Nacional (PEN) sobre VIH, ITS y TB 2012-2016. Un marco conceptual consideró varios factores determinantes de la prioridad política para la inclusión de inquietudes de salud sexual de mujeres que tienen sexo con mujeres en la elaboración del PEN en los años 2007 y 2011. Este artículo presenta los hallazgos de 25 entrevistas a profundidad con informantes clave y revisión de documentos, y destaca los resultados de la aplicación de categorías para un marco de los factores determinantes de la prioridad política para incluir asuntos de mujeres lesbianas y bisexuales en el PEN de Sudáfrica, tales como: poder de actor, ideas, contexto político y características de los asuntos. El artículo demuestra cómo los impulsores epidemiológicos y estructurales de la vulnerabilidad de mujeres lesbianas y bisexuales al VIH e ITS, tales como violencia sexual y otras violaciones de sus derechos sexuales, han sido expresados en foros políticos y si esto ha afectado la capacidad de mujeres lesbianas y bisexuales para reivindicar su derecho a la salud. … (more)
