- Abstract : Most man-made disasters do not happen because one single thing goes wrong, but rather as a result of the gradual accumulation of faults, errors and vulnerabilities that a healthy "safety imagination" can prevent. Reflecting on the impact of previous disasters in other sectors, from the Flixborough chemical plant blast of 1974 to the San Bruno pipeline explosion of 2010, this article charts how Grenfell Tower must now take its place in the roll of deadly but avoidable failures that, too late, sparked systemic overhauls in safety consciousness.
