Abstract

Abstract : Objective: 1) Compare blood pressure (BP) control treated based on impedance cardiography. 2) Compare the hemodynamic differences between groups. Design and Method: Inclusion: 50 to 75 y/o with ≥2 drugs and poor office BP control. A 24h ambulatory blood pressure monitoring (ABPM) study and impedance cardiography with Hotman® device were done. This system provides information such as cardiac index (CI), stroke index (SI), heart rate (HR), stroke systemic vascular resistance index (SSVRI), left stroke work index (LSWI), and thoracic fluids conductivity (TFC). Patients were divided in: control group (office BP > 140/90 and 24h ABPM < 130/80) and case group (office BP > 140/90 and 24h ABPM > 130/80). Only case group received treatment based on ABPM + CI, and both studies were repeated 4 weeks later. Results: Thirty one patients were included, 10 controls and 21 cases. Mean age was 65.7 ± 8.3 y/o. The 24h SBP was 121.2 ± 9.8 vs 143.6 ± 7.5 mmHg (control vs. case group pretreatment p < 0.001). Postreatment BP decreased to 130.6 ± 18.4 mmHg (pre vs. posttreatment, p < 0.001). The number of antihypertensive drugs was 3.2 ± 1 in control group vs 2.9 ± 1 in case group pretreatment (p = NS), and 3, 4 ± 1, 1 postreatment. The SSVRI was elevated in control group 229.8 ± 78 and decreased from 210.4 ± 80.3 vs 175.6 ± 55 FΩ/m 2 (control and pre vs. posttreatment, p < 0.04 respectively). Conclusions: Hypertension treatment based on impedance cardiography helped our therapeutic decision and improved BP control. The hemodynamic parameters normalized when HTN is treated according to IC. Further studies are needed to confirm these findings and its clinical relevance.
