- Abstract : Treatment of displaced proximal humerus fractures can be challenging, particularly in patients with osteopenia. Although large strides have been made with the advent of locking plate technology in the treatment of these fractures, severe fracture comminution and thin cortices can lead to loss of fixation and varus collapse. The use of an allograft strut, usually a fibular allograft, has been advocated in these fractures to help increase stability of the construct. In this report, we describe the use of a distal ulna allograft for reconstruction of proximal humerus fractures. Because of its anatomic curve, the distal ulna may aid in reduction and help prevent varus collapse. Surgical technique, procedure pearls, and results are described.
