No Improved Pregnancy Outcomes Noted in Women With Gastric Bypass Compared to Those With Class III Obesity [13D]. (May 2016)

Abstract

Abstract : INTRODUCTION: The purpose of this study was to compare pregnancy outcomes in women treated for obesity with pre-pregnancy gastric bypass to those with Class III obesity not treated with gastric bypass. METHODS: This study was a retrospective review of all women delivered between January 1, 2010 to September 1, 2013. The total patient population had an N=45, 961. Data was obtained from the electronic labor and delivery log. Group 1 included patients with class III obesity (BMI>40) at N=4, 314 patients and group 2 comprised patients that underwent gastric bypass with N=8. Statistics included chi squared & T tests. Statistical significance was a P value of <.05. RESULTS: Of the 4, 322 obese and gastric bypass patients reviewed, no significant difference was seen in important tested variables such as estimated gestational age (37.84 vs 39.13), gravida (2.71 vs 4.63), parity (1.08 vs 1.63), age (28.76 vs 32.13), BMI (45.164 vs 37.738), cord pH (7.25 vs 7.23), birth weight (3, 287.58 g vs 3, 507.75 g), APGARS at 1 and 5 minutes, cesarean section delivery (59.3% vs 87.5%), diabetes (12.1% vs 0%), pre-eclampsia (8.7% vs 0%), and NICU admissions (8.3% vs 0%). CONCLUSION: Our data did not show a statistically significant improvement in any measured variables in women who entered pregnancy with a history of gastric bypass versus those with Class III obesity. However, weight loss outside of pregnancy has been shown in the literature to be most effective in improving medical co-morbidities, especially diabetes and hypertension.
