Abstract

Abstract : Introduction: Tibetan patients live in high plateau areas and have special diet habits and customs. Only a few reports on Tibetan patients with intracranial aneurysm are published. In the treatment of these patients in our hospital, some unusual features were observed. Thus, our cases were collected, analyzed, and presented in this study. Patients and Methods: A total of 26 patients consecutively admitted to the Neurosurgical Department of West China Hospital, Sichuan University from September 2009 to March 2013 were studied. The patients were diagnosed with intracranial aneurysm either through computed tomography angiography or digital subtraction angiography. A total of 25 patients received interventional therapy or operative treatment, and 1 patient left the hospital without treatment. Results: Among the 26 patients with intracranial aneurysm from Tibet, 25 received treatment. Only 10 were found to have a single intracranial aneurysm near the circle of Willis. These 10 patients met the anatomical principles of common sites of intracranial saccular aneurysm. Among the remaining 15 patients, 4 had multiple intracranial aneurysms and 11 had a single intracranial aneurysm of unusual type. The patient who left the hospital without treatment had an unraptured intracavernous carotid artery aneurysm. 20 patients had subarachnoid hemorrhage before admission. No patient suffered from symptomatic cerebral vasospasm during hospitalization. Conclusions: A relatively high proportion of the patients had rare aneurysms, including pseudoaneurysm. Thus, risks in the treatment were remarkably high. Sufficient prediction and preparation are important to cope with unexpected intraoperative situations. The patients did not have a higher cerebral infarction rate under hypervolemic hypertensive hemodilution (3H) therapy.
