- Abstract : Objective: Uromodulin, the most abundant protein secreted in human urine is encoded by UMOD and its expression is restricted to the medullary region of the thick ascending limb of the loop of Henle. Common genetic variants in UMOD have been associated with renal function, blood pressure (BP) and hypertension. In this study, we investigated the influence of UMOD rs12917707-G>T on BP, renal and cardiovascular outcomes in a cohort of Caucasian patients with arterial hypertension and cardiac organ damage. Design and method: A cohort of 1218 treated high-risk patients (mean age 58.5 years, 83% males) with documented cardiovascular disease (81% with coronary heart disease) was analyzed. BP was determined by office and 24-h BP monitoring and cardiac parameters were assessed by echocardiography. Renal function was determined by estimated glomerular filtration rate (eGFR) while genotypes for UMOD rs12917707-G>T were determined by a TaqMan PCR method. Results: No significant association between rs12917707 and BP phenotypes was observed. A significant association between rs12917707 and eGFR was observed with T-allele carriers showing significantly higher eGFR values (+2.6 mL/min per 1.73 m 2, P = 0.006) compared to non-carriers. The SNP associated also with left atrial (LA) diameter (P = 0.007); homozygous carriers of the T-allele had smaller LA diameter (−1.5 mm) compared to other genotype groups (P = 0.040).
