Abstract

Abstract : Objective: The aim of this study is to analyse the incidence of hypertension and diabetes in patients with extracranial carotid artery stenosis compared to those with multisite atherosclerosis (MSA). Design and method: We examined all surgical patients with carotid endarterectomy in a ten-year period (2005–2015). We had 1055 patients: 773 men (73%) and 282 women (27%) of mean age 64.3 years (41–92 years old). We collected medical history and clinical data of the most common risk factors for atherosclerosis (smoking and dyslipidemia) and major comorbidities such as hypertension, diabetes, coronary artery disease and peripheral artery disease. Results: In patients with carotid pathology arterial hypertension was found in 96% of cases, diabetes mellitus - in 35%, smoking - in 66% and dyslipidemia – in 74%. Patients with MSA were 211 (20%), i.e. 1/5 of all patients had concomitant coronary artery disease and peripheral artery disease. In this group arterial hypertension was found in 100% of cases, and diabetes – in 81%. Smokers were 201 patients (95%), and dyslipidemia was present in 184 (87%). The average number of risk factors for atherosclerosis in all patients was 2.7 and in the MSA ones - 3.6. In the entire group unstable carotid plaques detected with duplex ultrasound were present in 645 patients (61%) and in the MSA group - in 134 (64%) (p = 0.09). Patients with asymptomatic carotid stenosis in the whole group were 41%, while in the group with MSA - 47%. In 59 patients we identified all four main risk factors for atherosclerosis as 19 of them had only carotid pathology while the remaining 40 of them had multisite atherosclerosis. Conclusions: Traditionally hypertension has high incidence as in isolated carotid pathology so in multisite atherosclerosis. In our study we have found very high incidence of diabetes in patients with multifocal atherosclerotic engagement - 81%. This raises some questions and concerns about its rapidly growing part in the development of the multisite atherosclerotic process.
