Abstract

Abstract : Objective: The lifestyle and the stress events can influence the blood pressure (BP) profile. Arterial hypertension and insomnia are two common conditions, however few studies evaluated the correlation between the sleeping quality and the prevalence of the arterial hypertension. Design and method: We enrolled 479 consecutive patients who performed the Ambulatory Blood Pressure Monitoring (ABPM) in our Hypertension Unit between November 2011 and September 2016. We collected the data about the anamnesis of insomnia (self-reported) and the use of the hypnotic drugs. We created two subgroups, one with a good sleeping quality and one with the people affected by insomnia, corrected or not by hypnotic drugs. We compared their general features and their BP profile evaluated with ABPM. Results: We compared the insomnia population (101 subjects) with the good sleeping quality one (378 subjects). The percentage of women was 64.4% vs 43.9% (p-value 0.0432). The mean age was 63.8 ± 12.0 years vs 57.4 ± 16.1 years (p-value 0.0124). The population with arterial hypertension at ABPM was 37.2% vs 47.6% (p-value 0.09306). The number of subjects in antihypertensive therapy was 84.2% vs 71.9% (p-value 0.0177). The number of antihypertensive drugs assumed was 2.4 ± 1.7 vs 1.8 ± 1.6 (p-value 0.0035). Mean systolic BP at ABPM was 124.7 ± 11.6 mmHg vs 126.5 ± 12.0 mmHg (p-value 0.1631). Mean diastolic BP at ABPM was 71.6 ± 11.6 mmHg vs 75.4 ± 10.0 mmHg (p-value 0.0003). The BP variability at day-time ABPM was 12.8 ± 3.5 vs 11.6 ± 3.1 (p-value 0.0039). There was no difference in the sleep length (p-value 0.9665) and in the type of the nocturnal dipping (p-value 0.1565). Conclusions: In our population people who referred insomnia had not higher prevalence of uncontrolled hypertension, however had higher day-time BP variability, greater probability to be in antihypertensive therapy and to take an higher number of antihypertensive drugs.
