- Abstract : Objective: Introduction: The aim of this work is to evaluate the effect of systemic arterial hypertension on the systolo-diastolic left ventricle function measured in patients with a hy in the acute phase of a first myocardial infarction. Design and method: 50 successive patients with a first acute coronary syndrome with a persistant ST elevation and admitted to the cardiology intensive care unit were included. All patients received coronary revascularization and Doppler ultrasound within 72 hours of hospitalization. High blood pressure (HA) was identified in 19 patients (38%). The ultrasound parameters are compared between the two groups with Hypertension (group 1) and without hypertension (group 2). Results: The mean age (57 ± 12 years vs 60 ± 12 years), blood glucose, cholesterol and triglyceride levels are similar between the two groups. However, blood creatinine level was higher in hypertensive patients (122 ± 50 micromol / l vs 91 ± 34 micromol / l), the treatment was later delayed in hypertensive patients (12 h vs 8 h, p = 0.01). there were no differences between both groups as regard to diastolic ventricular volumes (99 ± 33 ml vs 94 ± 20 ml) and systolic volumes (49 ± 22 ml vs 47 ± 12 ml) as well as the mean ejection fraction (50 ± 10% vs. 47 ± 12%). The systolic myocardial velocities were similar between both groups. Conclusions: CONCLUSION: In the acute phase of myocardial infarction, diastolic function disorders are more pronounced in hypertensive patients, whereas the systolic function evaluated by the ejection fraction and myocardial velocities show no differences in the presence or The absence of HTA. … (more)
