- Abstract: Open fractures and traumatic wounds represent a surgical challenge due to the risk of infection that can, at the extreme threaten life and limb but more commonly limit options for reconstruction. The treatment of these wounds requires removal of contamination and non-viable tissue in a way that minimizes further tissue damage, because healthy tissue in the wound bed is essential for microbial eradication. In more high-energy and complex wounds, particularly those associated with fractures, treatment requires a combined approach from both orthopaedic and plastic surgical teams. Initial surgical treatment is an urgent priority and must involve senior surgeons from both specialities. This combined approach can maximize potential for reconstruction and rehabilitation while minimizing risk of complications like infection and amputation. However, this combined approach requires significant resources and therefore these injuries should be managed in specialist centres appropriately staffed and resourced.
